The 25 Worst Hospitals in the United States Revealed

Hospital errors are responsible for 180,000 deaths each year, which works out to be 400 deaths per day. A new report on hospital performance in terms of infection rates, injuries, medical error and medication mishaps finds that some of the country's worst hospitals are concentrated in California, Illinois, and Texas, amongst others.

The report was issued by Leapfrog, a coalition of public and private purchasers of health insurance benefits. Leapfrog recently updated its list of poor-performing hospitals from June, doling out D and F grades to those that represent "the most hazardous environments for patients in need of care."

Four of the top five worst hospitals were located in California, with the biggest name among them being the University of California Ronald Regan UCLA Medical Center. Five hospitals from Illinois, and another five from Texas were also on the list of the nation's worst performing hospitals. Two South Carolina hospitals also made the list.
